• does not contain any promises of diagnosis, treatment, or cure

I acknowledge that in this process, the therapist:

• does not make decisions on my behalf

• does not offer guidance or guarantees

• offers ethical and limited accompaniment

I know that I am participating in the process of my own free will, taking responsibility for myself, and that I may be referred to relevant healthcare professionals when necessary.
